Patient Information
Your Physician remains the key member of our home care team and is responsible for overseeing the plan of care and ordering medications. In addition, our Medical Director oversees the team by reviewing all plans of care.  The Medical Director also serves as a resource for other physicians and clinicians.

Experienced Registered Nurses, certified in hospice care, visit patients to provide comfort.  Nurses visit with patients and support the caregivers, providing information and education.

Certified Home Health Aides assist patients with personal care, bathing, dressing and feeding, wherever a patient resides.

Social Workers provide guidance, education and counseling to our patients and families. The person with a serious illness has choices available to them that can have a significant influence on their quality of life.  Our Social Workers can help with these choices, and help patients and their loved ones adjust to the changes around them.  Social workers can also assist in coordinating outside resources as needed. Discussing your concerns and options can ensure that you are making the best decision.

The end of life often brings with it a keen desire for greater spiritual awareness.  Forbes Hospice has chaplains of all denominations available to assist patients with their spiritual needs.  Spiritual support is also available for loved ones.  Family members and caregivers are often stressed during the time of a loved one's illness.  When those surrounding a dying person are spiritually grounded, they can bring a great sense of peace and tranquility to their many hours of care giving.
