CMS Needs Additional Authority to Adequately Oversee Patient Safety in Hospitals
A new GAO report states that more than three-quarters of major deficiencies and errors at hospitals are not found during normal accreditation reviews.

Institute for Healthcare Improvement

Offers information regarding three critical areas of patient safety: medication systems, preventing falls and surgical site infections.  Includes the Safety Climate Survey tool which helps organizations measure the success of their patient safety initiatives.  Refistration is required, but free.

Institute for Safe Medication Practices
Provides independent review of medication errors that have been submitted to the Medical Errors Reporting Program developed by the US Pharmacopeia. Page offers safety alerts, drug safety product testing, and information on error reporting and electronic prescribing

Making Health Care Safer: A Crtical Analysis of Patient Safety
Covers 73 patient safety practices that are likely to improve patient safety, based on an analysis of published evidence, and describes 11 that the researchers considered "highly proven to work but not performed routinely in the nation's hospitals and nursing homes"

New Research Projects Awarded to Improve Patient Safety
Site provides detailed information on scores of patient safety programs funded by the Agency for Healthcare Research and Quality.  The projects address questions about how medical errors occur, in order to provide guideance on how patients, clinicians, policy makers and others can respond

The Patient Safety Act of 2004
Introduced May 17, 2004, by Hinchley to require Medical providers to disclose publicly staffing and performance in order to promote improved consumer information and choice.

Patient Safety First

Hosted by AORN.  Focuses on patient safety in surgical settings.  Stated mission is to help resistered nurses achieve the best outcomes for patients undergoing surgery.  Includes news, bibliographies, links, press releases and more.

Patient Safety in American Hospitals
This HealthGrades report is based on Medicare data from 2000 - 2002 and 16 of the 20 patient-safety indicators defined by the Agency for Healthcare Research and Quality (AHRQ).

Patient Safety Institute
An initiative to improve patient safety by offering providers secure, real-time access to an online patient database system at the point of care or decision. The five key areas of information: diagnoses, laboratory results, medications, allergies and immunizations.

Pennsylvania Patient Safety Reporting System

The Patient Safety Authority developed the Pennsylvania Patient Safety Reporting System, known as PA-PSRS (pronounced "PAY-sirs"), a secure, web-based system that permits healthcare facilities to submit reports of what Act 13 of 2002 defines as "serious events" and "incidents."  Statewide mandatory reporting through PA-PSRS went into effect on June 28, 2004.

Pennsylvania Medical Society

The Society offers online CME resources and two newsletters to help member physicians communicate better, reduce their liability risk, and improve relationships with patients, while earning CME that will qualify for the State Board of Medicine requirements for risk management and patient safety education.
